How we handle what you tell us through this site.
What we collect
When you send a message, we receive the name, email address, phone number and message you type. We ask for nothing about your health in the form, and we ask you not to include it.
How we use it
We use it to reply to you and to book a visit. We keep your records private and share them only as the law allows. We share information with a school or team only when a parent or guardian signs a written permission.
Who sees it
The front desk and the therapists who need it to reply. We do not sell your information.
Links to other sites
Our patient portal, bill payment and referral pages are run by other services and have their own terms.
Your choices
You can ask us what we hold about you, ask us to correct it, or ask us to delete a message you sent through this site. Call the front desk or write to us.
